The purpose of this form is to clarify and provide a step by step process in directing current and future team officials on the coaching requirements as dictated by Canada Soccer. This form is to be completed by the team official and actioned by the team official, based on the results.
If you are a team official for 2 teams please complete one form per team. The form will modify once you input your role and the age group to show specifically the requirements for being a team official on that team.
Each Submission you complete will be emailed directly to the email you provide so that you may reference this going forward as you work through your registration and training requirements.
Your submission is not sent to the zone office. This form is for your use only to ensure you have all the requirements for your role, and if not are able to action accordingly.
