Covid19 Vaccine

Certain customers of F2OnSite require individuals to be fully vaccinated against Covid-19. Please upload your CDC, State or Local vaccination record, document or other proof of vaccination in order to satisfy their requirements.

All records are securely held by Human Resources and are retained in accordance with Americans with Disabilities Act and Title VII of the Civil Rights Act. In certain situations, customers may require copies of these documents, and your signature below indicates your agreement to allow F2OnSite to share or submit your documents to our customer as required.

F2OnSite encourages our employees to become vaccinated against Covid-19 , wear a mask when in public or servicing customers, and to socially distance.
