Video Consultation Request Form

Thank you for your interest in Coastal Fertility Specialists. We hope you are doing well during these stressful times. We hope to provide you with something positive and productive during the quarantine period. If you are interested we are willing to offer you a 30 minute New Patient Video Consult for $75 until May 1st.  By completing the form below our new patient coordinators will contact you to get your information and set a time that is convenient for you and your partner.

During the consultation you will spend approximately 30 minutes by video with one of our reproductive endocrinologists reviewing your history. That will be followed by a discussion of recommended testing and treatment. Following that consult, on the same video screen, you will meet with your nurse coordinator and with a financial coordinator who will review your insurance coverage and costs. The entire video consultation is expected to last 45 minutes to an hour. The video consult will significantly accelerate your ability to start the testing immediately after the "lock down" is over. If you are unhappy with your video consult we will schedule you with an in person consult after the "lock down" at no additional charge.

For the video consult you will simply need a computer with a webcam running a chrome web browser and an internet connection. Details on the video connection will be sent to you by email.
