School of Public Health (SPH) DOVE Fellowship Nomination

This nomination form is for graduate research programs in SPH only. All other graduate research programs will apply through SLATE. Professional programs should apply to the Professional Education Diversity Fellowship.

If more than one faculty/staff person will be updating this form, you will need to share the username/password. The same username/password is used to submit multiple nominations. Do not share access with your nominees.

You may continue to revise nominations, even after submission, until the final deadline (February 5, 2025 at 5:00pm central time).

This nomination form must be completed by the Director of Graduate Studies or designated program staff (not by nominees).

Each graduate research program may nominate up to two (2) prospective students for the DOVE Fellowship.

The program confirms that they will provide the following REQUIRED funding for selected DOVE Fellows. Programs that do not confirm BOTH of the funding requirements below are not eligible for DOVE funding and the application will not be reviewed. *

$4,000 stipend plus insurance coverage (approx. $700 if needed) for each DOVE Fellow to participate in the GSDO Summer Institute (attendance optional for fellows).

A minimum of $1,000 for each DOVE fellow for research/conference travel to be used throughout duration of degree program.
