In connection with my application for service, I am giving my authorization to (First Baptist Cleveland or its representatives) to verify the information on this form. The church may contact appropriate government agencies to solicit background information relative to my criminal record history. I understand that First Baptist Cleveland may conduct inquiries into my background that may include criminal records, personal references, and other public record reports pertaining to me. Due to child protection policies, I understand that First Baptist Cleveland will be checking for a current background check on file, and that if there is not a current background check on file, they reserve the right to make further inquiries to ensure the safety of our students. After reading this document I fully understand its content and authorize the background verification. I also certify that the answers provided above and below are accurate to the best of my knowledge and belief. I am aware that failure to complete this application, intentional omissions, or misstatements may result in refusal of volunteer eligibility. Should my application be accepted, I agree to be bound by the Bylaws and policies of First Baptist Cleveland and to refrain from unscriptural conduct in the performance of my services on behalf of the church. *
