WAIVER, POLICIES, AND IMPORTANT INFORMATION

General Rules, Information, and Regulations

• All participants must be signed out by their parent/legal guardian each day. No person is permitted to leave the facility or pick up a child without written parental permission. 

• Appropriate sports-friendly apparel should be worn each day.   

• At times, there will be additional, nominal fees for some classes (For example, martial arts testing and equipment).  Your instructor will notify you and provide information for purchasing.

• Participants are to remain with their groups at all times.    

• Inappropriate behavior will not be tolerated.  If your child is asked to leave the program due to disciplinary reasons, there are no refunds.

• Please do not send sick children to the program. Parents will be required to pick up sick children. Staff is not permitted to administer medicine of any kind. If your child requires medical attention beyond a band-aid, parents will be called first if time permits, then EMT services.

• Bradstreet Sports, Inc. is not responsible for items lost, damaged or stolen at any program site. Children should not bring games and toys to the program.

• All fees must be paid in full prior to time attended. If a participant does not attend the program on certain days, fees are not pro-rated.

• Make sure your child has a healthy snack. Do not send food items that need to be refrigerated or microwaved. Please put your child’s name on all personal items. Bradstreet Sports does not provide snacks for this program.

• Tutoring and homework assistance is to aid in completion of homework.  We do not guarantee that all homework will be completed nor that homework will be without errors.  We make no guarantees about your child’s academic success.  The purpose of our homework assistance and tutoring component is to assist students and provide a setting for group tutoring.

• Have your child wear comfortable clothes and shoes so that they can participate in active classes comfortably and safely.

• Parents or guardians or other authorized person must sign their child out each day. Do not ask them to meet you at another location. If you arrive late or plan on picking your child up early, please check in or out with the staff at the program. Note: Any parent or authorized individual picking up a child after pick up times will be charged a $5.00 late fee per child for each 5 minutes late.  This policy will be strictly enforced.

• If you send money or personal items with your child, Bradstreet Sports, Inc. will not be held responsible for lost money or personal items. Participants should not bring toys, material items, tablets, electronic games or an excessive amount of money that could easily be misplaced lost, broken or lost.  Participants may not use electronics of any kind during the programs/classes.

• If there is an emergency or you need to contact your child, call 1 877 237 8419. A staff member will be able to get the message to the program director.

• Program and classes will not operate during any school holiday, vacation day, or teacher work day.  Programs will be available on early release days.  No refunds, credits, or makeups will be made for these non-operational days nor are there prorations or reduced charges for partial weeks.   If you participate in the best rate program and wish to stop recurring payments for a particular week for any holidays, you will be charged the day rate of $35 per day for any days attended during that week.  

• Payment, terms, and or other policies are subject to change.  We will notify you in advance of any changes.  You may cancel or withdraw at any time (you may lose your spot if you withdraw or cancel).

• Customers are free to switch from one billing plan to another plan (prepaid days to recurring or recurring to prepaid) but must first finish out their current plan before switching.  In other words, you must use the remainder of your prepaid days for prepaid plans before switching. For recurring plans, you must finish your current week before switching to a prepaid plan. No changes may be made until your current billing plan is complete.

• Payment must be submitted every Monday for the following week's programs.  Late payments are subject to an additional $15 fee.

• For Best Rate participant, if your child is sick, on vacation, or missing due to appointments, etc., normal fees apply and no credits or refunds are issued.

• Prepaid days expire at the end of the current school year.  No credits or refunds are issued for unused days.

• Your initial payment and registraion fee are processed online via credit or debit card.  All future payments must be made by check or money order.  If you are on the best rate program (weekly), your payment is due each Monday for the following week's programs.  If you are a prepaid 10 or prepaid 20 plan, we will do our best to notify you when you are close to running out of prepaid days but it is your responsibility to maintain records and pay when your days are running out.  If you run out of days and are not prepaid, you are subject to the daily rate of $35 per day for after school and $65 per day for summer or other camps.

Transportation Permission

The following transportation release is required for those needing transportation from a local school to the host site.

My child has my permission to ride and be transported in a vehicle provided for transportation by Bradstreet Sports, or any related entity, agent or contractor. 

I understand that health insurance is required to participate in any programs.

If I am using transportation services, I agree to notify Bradstreet Sports if my child will not be attending by 11 am on the day of the absence.  If I fail to notify Bradstreet Sports by 11 am, I understand that there will be a $12 fee assessed.

Refunds are available prior to the start of the program by written request and program receipt, less a 25% administrative fee. Once the program begins there are no refunds.  

Conduct: Participants are expected to have lots of fun! However, they are also expected to observe appropriate conduct at all times. Conduct reflects upon themselves, their family, and the Bradstreet Sports, Inc. philosophy. Failure to comply with academic and behavioral expectations will result in push-ups, laps, cleaning details, or other appropriate discipline. 

IN CASE OF EMERGENCY, I/We authorize Bradstreet Sports, Inc. in whose care my child has been entrusted, to take my child to an emergency room, doctor’s office, clinic or hospital via EMS or other official emergency services. I/We also give my/our consent to an x-ray examination, anesthetic, medical or surgical diagnosis or treatment, and hospital care, to be rendered to my child, by a doctor or surgeon licensed to practice in any state in the United States. I HAVE READ THIS FORM AND I AGREE TO OBSERVE ALL THE GUIDELINES LISTED ABOVE. I understand that there is an inherent risk in my participation in this program, and I agree to hold harmless Bradstreet Sports and any School and other related entities and I agree to hold harmless Bradstreet Sports, Inc. and any employee, or independent contractor of Bradstreet Sports, Inc. against any and all claims resulting from my participation in these programs.

Specific Do’s and Don’ts:

Participants must: • Have fun, yet listen, respond and react appropriately to counselors and instructors. • Perform to the best of their ability in all activities. • Strive to be a positive influence on others. • Bring any problems with other participants to the attention of their coach or instructor in. • Stay with their groups AT ALL TIMES.

DISCIPLINARY/BEHAVIOR INFORMATION

Dear Parents: Proper behavior and cooperation is essential in this program for the safety and enjoyment of participants and staff. The following policy has been established to ensure that all parents and participants understand how our staff will handle situations as they arise. Should you have any questions or concerns, please do not hesitate to let us know. We will do our best to be fair and consistent.

 “ 3 STEP POLICY” FOR DISCIPLINE • “COOL OUT”- CHILD WILL BE SET ASIDE FROM THE ACTIVITY HE/SHE IS PARTICIPATING IN (for a period not to exceed 10 minutes). THE CHILD AND CAMP DIRECTOR WILL TALK ABOUT THE BEHAVIOR TO MAKE SURE IT DOES NOT HAPPEN AGAIN. PARENTS WILL BE NOTIFIED OF SUCH BEHAVIOR. • A WRITTEN REPORT ON THE REPEATED BEHAVIOR IS GIVEN TO THE CAMP DIRECTOR. THE PARENTS WILL BE NOTIFIED FOR THE SECOND TIME TO DISCUSS THE BEHAVIOR OF THEIR CHILD. • SHOULD THE BEHAVIOR CONTINUE, THE PARENTS WILL BE NOTIFIED A THIRD TIME AND BROUGHT IN FOR A MEETING. AT THIS TIME, AS A LAST RESORT, THE CHILD MAY BE EXPELLED FROM THE PROGRAM. These programs will be a great experience which will positively affect your child’s present and future. It is necessary to have a discipline policy in place to ensure a quality program for all participants. Please review the importance of conduct with your child(ren) and our discipline policy. I UNDERSTAND AND ACCEPT THE DISCIPLINE/BEHAVIOR POLICY. 

I/We have legal custody of the participant listed above a minor, whose age is listed above and born on the date listed above.

I/We give the individuals listed in the "Parent Authorization for Pick Up" section permission to pick up my child:

Medical Permission and Release Form In consideration of being permitted to participate in Bradstreet Sports, Inc. programs, I hereby authorize Bradstreet Sports, Inc. and its staff to see that my child(ren) listed above receive appropriate medical attention as the staff deems necessary. I hereby release Bradstreet Sports, Inc., owners, and its staff (hereafter, referred to as *Bradstreet*) from any and all claims arising from damage or injury resulting while participating in this activity.

*In case of emergency, I understand that every effort will be made to contact me. In the event that I cannot be reached, I hereby give my permission to Bradstreet Sports, Inc. to secure proper care. It is understood that I, as parent/legal guardian, will accept financial responsibility for the payment of any emergency transport, care, physician, or any hospital expenses or other expenses that might be required. In exchange for the right to participate in the instructional programs, special events or other events (*programs*), I agree to indemnify, hold harmless and defend Bradstreet Sports, Inc., its officers, agents, servants, and employees and staff from any and all claims resulting from injuries, damages, losses, or death sustained by me or my children and arising out of, connecting with, or any way associated with the activities of this program.

*I understand that there are certain risks involved in participation in these programs and I hereby assume such risk. These include, without limitation, the risk of physical injury, mental injury, emotional distress, trauma, death, contact with other participants, and the effects of weather, including extreme temperature or conditions. This release discharges Bradstreet and all other promoters, sponsors, officials, hosting entities such as clubs, churches, and any municipality or public entity or homeowners* associations and their respective agents and employees (*entities and related people*) associated with the programs from and against any and all liability arising out of or connected in any way to the programs, even though that which may arise out of negligence, carelessness, gross negligence or intentional act of Bradstreet Sports, Inc. entities and related people or others or participants. Participants and parents or other representatives of participants hereby release, and promise not to sue Bradstreet Sports, Inc. or its owners and also agree to indemnify Bradstreet Sports, Inc. and entities and related people for any such injury whether such injury arises from the negligence, carelessness, gross negligence, or intentional acts of Bradstreet Sports, Inc. entities and related people including but not limited to all cost associated with any injury, including attorney*s fees. The above paragraph also applies to Imagine School at LAND O’LAKES LLC, Imagine Schools Nonprofit, Inc., Imagine Schools, Inc., and Schoolhouse Finance, LLC.  

*The parent, guardian and participant hereby grants full permission to use any photographs, videotapes, recordings or any other record of these programs in print media, internet advertisements, or websites, television, radio, or any other public forum.

*The parent, guardian and participant agree that this waiver and release shall be binding on participant*s personal representatives, heirs, assignees, and next of kin.

*The parent and/or guardian have carefully read and understand the foregoing waiver and release and sign the release as the participant*s own free act and that by signing the release, the participant makes a total and unconditional release of all claims against Bradstreet Sports, Inc. The participant understands that this release is intended to be as broad as allowable by Florida law and if any portion of this release is deemed invalid, the remainder shall maintain its validity and be applicable. The participants represent that in executing this document, the party doing so has full authority to enter into this document with Bradstreet Sports, Inc. on behalf of themselves and acknowledge that Bradstreet Sports, Inc. is relying on the execution of this document in allowing the participants to participate in these programs.

Bradstreet Sports does not provide transportation for students from school/classroom to the program. For further details, check with your site director!
