The Department of Human Services' Premier's Certificate of Recognition for outstanding volunteer service recognises the exceptional contribution of South Australian volunteers who deliver many critical community services in the areas of youth and community development, welfare, education, emergency services, sport and recreation, the environment and more.

Thank you for taking the time to nominate an individual or team for the certificate program. Before you get started on the nomination form, please read the guidelines to make sure that you have all the necessary details to complete the form.

As all certificates will be printed as per the details supplied on the nomination form, please make sure that you double check the spelling of the nominee's name/ team name/ organisation etc. before submitting.

Part 2: Individual or team nomination?

The Premier's Certificate of Recognition for outstanding volunteer service is a prestigious South Australian volunteer award. As such, certificates are awarded on a limited basis to a select number of volunteers.

Individual nominations are limited to eight (8) per organisation or community group.

Organisations or community groups recognising more than eight volunteers can do so by nominating them as a team - only one certificate will be issued on behalf of the team.
