Festival Vendor Application Form

FRIDAY 25TH: 5PM - 11PM (FIREWORKS)

SATURDAY 26th: 1PM – 11:00PM (FIREWORKS)

SUNDAY 27th: 12PM – 9:00PM

MANDATORY: All vendors must provide a special event certificate of liability insurance naming the Greek Orthodox Church of the Assumption, 430 Sheep Pasture Rd., Port Jefferson, NY 11777 as the certificate holder. If you need any assitance please contact the church office at 631-473-0894. 

*IMPORTANT: OUR PARKING LOT EXTENDS INTO THE VENDOR AREA. THEREFORE YOU WILL NOT BE ABLE TO USE STAKES TO SECURE YOUR TENTS. PLEASE BRING SOME TYPE OF WEIGHTS (CINDER BLOCKS, SANDBAGS, BARBELL WEIGHTS) TO SECURE YOUR TENTS. THANK YOU

The church will provide general lighting. Vendors must provide their own displays. ALL TENTS MUST BE FIREPROOF/FLAME RETARDANT!  THE CHURCH IS NOT RESPONSIBLE FOR ITEMS LOST, STOLEN, OR DAMAGED.   For information email: goc.assumption@gmail.com Subject line “VENDOR”

FEES: Are not refundable. Must be paid in full with this contract. Any fee paid after Auust 1st must be cash or certified check or money order and is also subject to a late filing fee of $75. Make checks payable toGreek Orthodox Church of the Assumption”. Mail to 430 Sheep Pasture Road, Port Jefferson, New York 11777.

When your application is approved and accepted, you will receive a receipt as “confirmation of your reservation”. Space is limited. Spots will be determined approximately 1 week prior to the start of the Festival and you will be notified of your spot number. (Office Use:­­­­                              )
