Dear EOP Scholar:

This application is for consideration to receive funding during the 2014 summer session.  Please note that in order for your application to be considered, you must adhere to the following procedures:

1. You must complete all sections of the application.  Please note that an incomplete application will not be considered.

2. You must have met all of the requirements of the EOP scholars program (i.e. attendance at all scheduled community meetings, completion of structured study hours, meeting with SDS as scheduled, and submission of the EOP student contract each semester).

3. You must have filed the Free Application for Federal Student Aid (FAFSA) Form for the 2014- 2015 Academic Year AND have submitted to EOP a copy of your parent's or guardian's 2013 Income Tax Transcript.

4. You must submit the completed application by April 18, 2014. Please note that your application cannot be processed until all supporting documentation is on file. It is your responsibility to ensure that your FAFSA is completed, that your parent's or guardian's 2013 tax information have been submitted to EOP, and that you have submitted your EOP student contract each semester. Additionally, your community meeting attendance, your structured study attendance, and your student-SDS meeting attendance,  during all requested appointment times, will be evaluated to determine your eligibility for summer funding.
5. You must be in good standing with both the University and the Educational Opportunity Program (EOP).

6. If you have failed to adhere to any of the above procedures, you will be disqualified for any summer funding from EOP.

Summer Housing Funding Request

Please Note: You can not request funding support for housing and allow someone else to live in your room or co-reside with you unless they have paid for the ability to reside within the room with you.

Statement of Recognition

As an EOP Scholar, I recognize and agree to all of the following items:

-          I must register for all appropriate courses before my award can be placed on my account.

-          I CANNOT receive any refunds from any funds received for my summer academic efforts. If I find that I may be eligible for a refund check, I will speak with the Program Director, prior to requesting my refund.

-          I will adhere to all the rules and policies of Seton Hall University and its respective divisions.

-          If I fail a course, withdraw from a course, or are found to have missed more than 2 class sessions, my summer award may be revoked immediately. This can result in my need to pay for both my course(s) and any other items (if applicable).

-          I will meet with my EOP Counselor and seek academic support through both CLIA and any other academic support services office at Seton Hall to ensure my continued academic success throughout the summer.

-          Any award received for the summer CANNOT be applied to any owed balance from a previous semester or be carried forward for any future semesters.

-          In addition to my SDS, I must notify my EOP Program Director and/or Associate Director of Counseling and Enrollment Services of any changes to my summer academic plans or needs within 24 hours of determination.

-        I recognize that by submitting this application that I am not gauranteed summer funding and I will seek financial support through alternative means (student loans, grants, etc.) to assist me with my summer academic endeavors.

-        If I am awarded funding, I must notify the EOP office within 48 hours of my intent to use my award. If I fail to notify the office of my intent to use my award within 48 hours, my award can be revoked and given to another student.

-        If awarded summer funding, I must come into EOP (within three days of having received my notification) and MUST submit a summer student contract and meet with the Director and my SDS to discuss my summer plans. We will also review  the policies and rules,  governing  acceptance of my award.
